Admissions

We provide care to all residents without discrimination. Resident accommodations and treatments are provided equitably, with consideration given to meeting individual cultural, psychosocial, and accessibility needs.


The Process:


Initial Inquiry 

Prospective residents or their family members may inquire about assisted living services through phone call, email, or visit to the community. We provide them with information about our available services, fees/costs, and entry requirements.


Screening & Needs Assessment

To determine if we can adequately address their care requirements, we perform an initial assessment of potential residents either via phone or in-person. During this review, we gather key details regarding the individual's health status, ability to move around independently, capacity for self-care, and any other assistance that may be necessary for their activities of daily living and their personal preferences. Compiling this information allows our admissions team to decide if the services and amenities within our assisted facility would be suitable for the applicant.


Application Form 

In order for us to review eligibility for admission, we kindly request that potential residents or their designated representatives fill out the application forms 


Completing this paperwork allows our team to evaluate if our assisted living community can appropriately serve the needs of the applicant. We encourage interested parties to submit these materials so we can determine next steps in the admission process.


Community Visit & Assessment  

As a vital part of qualifying for acceptance into Doc Hill's Family Care Home, we arrange an extensive one-on-one appointment with potential residents. This in-person meeting allows us to:

  •  connect directly with the prospective resident, 
  • take them on a complete tour of our compassionate community, 
  • perform a comprehensive hands-on evaluation of their care and mobility needs


Review & Approval  

After collecting the prospective resident’s application information and completing in-person evaluations, our compassionate directors carefully examine all materials compiled during the admission process. Leveraging their expertise and consideration, they officially decide on acceptance into our warm community. 


Move-in Preparations 

When a prospective resident is granted admission into our nurturing community, our dedicated team partners closely with them and their loved ones to facilitate a seamless transition. We aim to alleviate any anxieties.


Upon admission the following items are needed:


  • Identification card(s)
  • Current completed FL-2 
  • Insurance cards and policies
  • TB Test
  • Military discharge form DD214 or equivalent
  • Health surrogate appointment
  • Medicare or Medicaid cards
  • Living will, power of attorney, or durable power of attorney
  • Identification card of Power of Attorney
  • Copy of an organ donation request card
  • Copy of Do Not Resuscitate
  • Physician's Medical Evaluation from the past 6 months
  • Mobility or Assistive Equipment Documents if applicable

Personal Items Notification

While we aim to provide a safe and secure environment, Doc Hills Family Care Home is not responsible for any loss of or damage to residents' personal belongings. We encourage those staying with us to limit items brought into our facility to those necessary during their stay.


Residents may bring personal items into Doc Hills Family Care Home at their own discretion and liability. We will make reasonable efforts to provide secure storage for valuables and belongings within a resident's room. However, we cannot guarantee the security of any personal possessions. It is the responsibility of the resident to secure and track their own belongings. 


We ask that any valuable personal items, such as electronics, jewelry, sentimental items, etc. remain at residents’ primary residences if possible. Doc Hills Family Care Home does not assume liability for the replacement or repair of any lost, damaged, or stolen personal belongings.
